It's a GM error, and of course, Shanahan is the GM. While I find Shanahan the coach adequate, the GM has been subpar. So him calling a timeout instead of a challenge (which in the worst case scenario ends up being a timeout) when the QB's knee touched the ground before he pitched it, isn't a coaching error?
